Is it time for assisted living?
Seven questions. Two minutes. A clearer picture of where your family actually stands, and whether it is worth a conversation.
Is It Time for a Care Change?
Seven questions. Two minutes. A clearer picture of where your family stands.
Most families aren't sure when to act. They wait because the situation is murky, because the conversation feels too hard, or because they're hoping things settle on their own.
This check-in won't tell you what to do. It will tell you what an advisor would notice, and whether it's worth a conversation.
Your answers stay on your device while you take it. Nothing is sent unless you choose to ask an advisor to reach out.
The six signals that come up again and again
A fall, or a near miss
One fall roughly doubles the odds of another. The one you hear about is usually not the first.
Medication going wrong
Doses skipped, doubled, or quietly taken over by a family member. This is often the earliest concrete signal.
Memory changes
Repeated questions, missed appointments, food left to spoil, or confusion that arrives in the late afternoon.
Daily tasks slipping
Bathing less often, wearing the same clothes, a kitchen that has stopped being used.
A caregiver at the end of their rope
Burnout in the spouse or adult child providing care is one of the most reliable indicators that a change is needed.
Safety scares at home
The stove left on, a door left open, wandering, or being locked out. These rarely stay isolated.
If any of this sounds familiar, our guide to the levels of care explains what each option actually provides.
About the check-in
Is this check-in a medical assessment?
No. It is a set of questions our advisors ask families every day, scored so you can see where you sit. It is not a diagnosis and it does not replace a clinician. What it does is turn a vague worry into something specific enough to act on.
What happens to my answers?
Nothing, unless you choose to send them. Your answers stay on your device while you take the check-in. At the end you can ask an advisor to reach out, and only then do your details and your result go to Placita.
How long does it take?
About two minutes. Seven questions, three options each.
What if my score says "stable for now"?
Then that is good news, and no one will chase you. Steady is not the same as no plan needed though, care situations change quickly, and the families who handle this best are the ones who understood their options before they urgently needed them.
Does taking it commit me to anything?
Not at all. There is no cost, no obligation, and no mailing list. If you would rather just talk to someone, call (520) 771-5919 at any hour.
